In Cameroon about two out of five children under 5 years, are not registered at the civil registry office (CRO). Methods: A community-based cross-sectional analytic study was conducted from November 2021 to May 2022 among 414 households selected by stratified random sampling. We studied knowledge on birth registration, and the registration status children in the household. Basic statistical tests were used to study the association between variables. Results: Findings reveal that, 172 (41.5%; C.I at 95% [36.7- 46.5]households had unregistered children. The significant predictors of this poor birth registration where: poor knowledge on birth registration (aOR = 2.4; C.I at 95% [1.5-3.8]; p<0,001), less than five children in the household (aOR=4.4; C.I at 95% [2.5-7.6]; p<0.001), and mother’s age of 24 years and less (aOR=5.0; C.I at 95% [2.4-10.7]; p<0.001). Conclusion: Birth registration is suboptimal in Yaoundé II; three factors were found to be associated with poor birth registration.
